Student Health Insurance coverage
is MANDATORY for all full-time undergraduate (12 credits
or more) and graduate students (9 credits or more), and will AUTOMATICALLY be charged to the student's account unless properly waived. U.S.
government guidelines require health insurance coverage
for both full-time and part-time International students,
including coverage for all dependants residing with them
in the United States.
For the 2008-2009 Academic
Year, the University's Student Health Insurance Coverage
begins August 10, 2008 and ends August 10, 2009. Domestic
students who are registered for 6 or more credits may choose to purchase student health insurance
by checking the ENROLL option on the Student Health Insurance
Enrollment and Waiver Form (Campus Services Office). The form must be completed by the appropriate open enrollment cut off
date for each semester.
Students seeking to enroll
after the open enrollment period must provide documentation
of involuntary termination of previous health insurance
coverage within 30 days of insurance termination.
Florida Tech has teamed with Markel
Insurance Company to provide a user-friendly student health
plan. Filing a claim is as easy as logging onto www.studentassist.com (password 0277).
In addition to an on-line claims form, the site also features
a Frequently Asked Questions (FAQ) section about the claims
